If you have been discharged from hospital and have been provided with new medication which is not available via your NHS app or Patient Access app, then please DO NOT call the surgery. Instead please contact the Practice using via an eConsult request
If we need to clarify the medication request we will contact you.
Please be aware that any new medication started by a hospital following an admission requires the Practice to be notified in writing (e.g in the form of a discharge letter). This letter needs to be added to your records and for a member of the Practice team to review such documents before updating your medication. You should therefore ensure that you have been provided with enough medication until we have recieved this letter and an additional 10 working days to process any correspondence.
